Across Ontario, there have been many excellent examples of how health service providers, community organizations and LHINs have come together to address health equity concerns in their local communities. Below you will find some examples of initiatives that have had significant impact on addressing health equity and population health.
The Mississauga Halton LHIN hosted its second annual Health Equity Symposium on March 23, 2015. The focus of the symposium was to provide a better understanding around the why, how and what behind collecting socio-demographic client data.
